The International Classification of Sleep Disorders (ICSD) is a comprehensive clinical resource for diagnosing sleep disorders. The most recent edition, the ICSD-3-TR, was released in June 2023. It organizes sleep disorders into six major categories:
There are over a hundred sleep disorders classified, however the most common are: Obstructive Sleep Apnea, Central Sleep Apnea, Insomnias, Hypersomnia's, Restless Leg Syndrome, Periodic Limb Movement Syndrome, Narcolepsy, Shift Work Disorder, REM sleep Behavior Disorders, and Advanced/Delayed Sleep Phase Disorders.
The American Academy of Sleep Medicine has a comprehensive list of sleep disorders and information for patients and their families. Follow the link below for great information on sleep disorders
A Polysomnography sleep study provides a sleep doctor with the most complete evaluation of your sleep. You will be required to stay overnight at a sleep center. A sleep study records your brain waves, heartrate and breathing as you sleep.
A home sleep apnea test provides a sleep doctor with information to diagnose obstructive sleep apnea. It allows you to sleep at home wearing equipment that collects information about how you breathe during sleep. You will usually set up the testing equipment yourself.
In this example we see the baseline pre-treatment portion of a PSG study. Looking at the lower breathing channels we see frequent breathing pauses separated by a group of large rapid breaths and pauses again. This cyclic pattern is common in more severe sleep apnea cases. Note the Oxygen drops with each breathing pause and the noise in the brain waves in the top channels with each breathing pause.
In this example, we see the same patient treated with CPAP therapy and the breathing pauses have been eliminated and breathing is smooth and regular. Oxygen saturations are now stable, and the brain wave activity is more relaxed. Treatment of sleep apnea, regardless of therapy device should strive for similar results.
Positive Airway Pressure is a "first line" treatment of Sleep Apnea as it is quick, and results can be seen quickly.
Dental Appliances are very popular for helping treat Sleep Apnea and Bruxism. Dentists may be Board Certified in Dental Sleep Medicine
Many surgery or implant options may be available for various sleep disorders. Surgeries can be extreme or minor outpatient procedures.
CBT-I teaches individuals to identify and replace dysfunctional beliefs about sleep with positive thoughts. CBT-I is directed by trained professionals (psychologists, psychiatrists, or another medical doctor) with experience treating sleep disorders.
Sleep Hygiene is critical to keeping healthy and limiting your chance of getting yourself into trouble with some basic sleep problems.
